臨床における問題解決能力は単一の包括的な能力ではない。異なる症例問題を用いて臨床能力を評価した場合に、設問間の評価の相関係数は0.1から0.3程度に留まることが多い。このことからわかるように、各症例にはそれぞれ特徴的な課題や要点が含まれている。それがkey featuresである。臨床能力を、妥当性も信頼性も高い方法で評価しようとするならば、30-40例の症例に関するkey featuresに焦点を絞ることが極めて有用である。この講義の目的は、"Key features"の概念を紹介し、カナダの医師国家試験の一部にもなっているその理論と実践を紹介することである。Key featuresの問題作成に関する様々な観点を、具体例や調査データを用いて呈示するとともに、他の評価方法と比較した場合の利点についても述べる。Key Features法は臨床決断能力だけでなく、診療技術の評価（たとえばOSCEなど）や、基礎医学や臨床医学の知識の評価（MCQ試験）などにも応用可能である。
＜英文＞
Clinical problem solving is not a generic ability. Inter-case correlation of performance is typically in the 0.1 to 0.3 range. Thus, each case presents unique challenges or critical elements, the case's key features. Assessment of clinical decisions is maximized by focusing on the key features for a wide range of cases, some 30 to 40 cases, in order to obtain valid and reliable test scores. The purpose of the presentation is to introduce the notion of "key features," its rationale and implementation as part of Canada's national qualifying examination in medicine. The various aspects of developing key feature problems will be presented along with concrete examples and research data. Finally, the Key Feature approach will be compared to other assessment methods and advantages will be highlighted. The Key Feature approach can be applied, not only to assess clinical decisions, but also for clinical skills (e.g., the OSCE, Objective Structured Clinical Exams) and basic science and clinical knowledge (e.g., as in MCQ (multiple choice question) exams).
